Step 1: Before Travel
Click HERE to apply for permission to enter Sarawak

Step 2: Click HERE and submit your eHDF
The eHDF has a validity of 12 hours, please only submit this within 12 hours of your expected arrival time.

Step 3: Upon Arrival
At the port of disembarkation in Sarawak, show your QR code page to the relevant authorities as needed.

Submit eHealth Declaration Form (eHDF)

The eHDF has a validity of 12 hours, please only submit this within 12 hours of your expected arrival time

Step 2: After submitting your eHDF, show the generated QR code page to the relevant authorities as needed

*For those travelling between Zone 1 (Kuching, Serian, & Samarahan) and Zone 2 (all other districts in Sarawak) please apply for POLICE PERMIT prior to your travels. The police permit application form can be downloaded HERE and can be submitted for approval at your nearest police station.
